HOW TO GET A COI FOR THE 2025 SEASON

STEP 1: COMPLETE OUR MEMBER ORGANZATION INTAKE FORM

It is a very simple form that enables us to verify you in our system. Once you have completed the form you will receive an autoreply email to confirm the submission went through. If we have you in our system, you will be contacted by your Regional Coordinator and sent Step 2 (the 2025 Member Organization Agreement). If you are not in our system, your Regional Manager will connect with you to get you started.

STEP 2: COMPLETE THE 2025 MEMBER ORGANIZATION AGREEMENT

This is an online form that acknowledges that you have reviewed and agree to abide by our member organization agreement and requirements.

Here are the requirements:

• All your players and coaches will be active members through the entirety of your season
• Your program will upload all your players and coaches into our USA Lacrosse Roster management tool no later than 30 days after registration closes
• All your coaches are Bronze certified
• At least one Program Leader has an active Program Leader membership type

Once you’ve signed the 2025 Member Organization Agreement you will receive an autoreply email as confirmation – this will contain the link to the form that will allow you to request a COI (see step 3).

Please note – if marked on the initial intake form that you are a new Program Leader, you will also receive an email with information on additional opportunities and services we provide that can support both you and your program's success.

STEP 3: COMPLETE THE COI REQUEST LINK

The COI request link can be found in the autoreply email you will receive shortly after submitting the 2025 Member Organization Agreement. You will receive a COI no later than 24 hours after submitting the COI request.
